As part of your services’ safe sleep practice, a risk management plan should be prepared to show how you have identified and manage the risks for the children in your care. You need to be able to show how you have considered each individual child and that you have tailored your sleep and rest practices to accommodate their needs.
This detailed multi-page Risk Assessment Template and Guide has been prepared with a focus on safe sleep for infant children in the 4-11 month age group. It includes references and information for your general knowledge, as well as identified risks and ways that you can manage these.

The Risk Assessment Template and Guide is one of four main documents in the pack you will receive. Second is the Checklist, which will help keep you on track. The other two documents are templated forms to help you understand the children’s needs and the families’ expectations, and how you will accommodate them.
Should you also provide care to newborns (0-3 months), toddlers (1-2 years) and/or preschool aged children (3-5years), you will need to have a separate risk assessment for how you manage these age groups.
